Date that service was provided, not date entered by staff where different.

Status STVSSST:

The Student Service Status Validation Form (STVSSST) is used to create, update and delete codes and descriptions to define the status of the service.

Note

Doesn’t affect reporting changes.

Example: Service Code that crosswalks to “Yes, service provided” is not negated by a Status of “Service Refused”

Provider Origin STVORIG:

What department/area provided the service to the student. For the purposes of this, it may be as simple as what internal department/area was the student sent to for the service?

For example, if a student was forwarded to the LRC for technology equipment I’d expect them to be listed.

Service Group STVSSGP:

Has a combination of goals, needs, and/or services that may be assigned together.

A person may be associated with multiple Service Groups.

Goals, needs, and services are all independent of each other. A person may have a goal without a need, or have a need without a goal, or have a service without a need or a goal.

For example, if the CalFresh outreach was a selection of students with that same goal or need then they could be put in as a Service Group.
